My best pacifist runs I had an early flamethrower (pre-9).
You want to start killing some stuff at some point, because the amount of enemies on the screen and thus free stuff from passive is capped.
Elemental damage is also the best to eventually spec into, because you don’t tank it by taking harvest items (except wheat I guess, but its epic rarity).
Medical_gun_ is garbage at lifestealing, your better bets are either medical saws or flamer/minigun/smg + lifesteal stat.
You can get some lifesteal from mice, actually buying real items for it seems like a waste.
Elites and bosses you definitely just run around and pray still.
UPD:
I tried another pacifist run and got wave10 flamethrower + 2 early hordes which normally would have been a disaster.
Didn’t record it, but it was a funny one, ended with 4 flamethrowers, a taser and a random-ass nuke launcher. And +35% enemy count because having more lag from everything slowly burning is always fun.
Also, maybe hands are a bait.
They seem like a helpful thing to have early on, but then when mobs start spawning from all directions, hands keep knocking them back on your own move path. Ideal amount of knockback to have on weapons is 3 to 8 — enough to keep mummies off your tail but not enough to turn a problem into a bigger problem half a second later.
